Why the forms
The paperwork, without the paperwork
No copying numbers onto a PDF, and no chasing signatures by email.
The BACB forms, filled in
Name, BACB ID, hours, supervision, and supervisor, carried from your entries onto the form.

Draw, type, upload, or reuse a saved signature. The form then waits on your supervisor's dashboard.

Common questions
Which forms can I sign in the app?
The BACB monthly (M-FVF) and final (F-FVF) fieldwork verification forms, in the 2022 and 2027 versions, filled in from your logged hours. RBTs sign ABA FT's Monthly Supervision Record.
How does signing work?
You sign by drawing, typing, uploading, or reusing a saved signature, then send the form to your supervisor, who signs it on their account. You can also email the signed PDF or print it for a handwritten signature.
Does my supervisor need an account?
To sign in the app, yes, and it is free. Invite them from the form, and they connect when they sign up.
What if a month does not meet every requirement?
The form shows a clear flag, but nothing is blocked. Whether to sign stays your decision and your supervisor's.
What happens to a signed form?
It locks once your supervisor signs and stays available to both of you, even if you later disconnect.
