Start Date and End Date
The first and last month of fieldwork this form covers (MM/YYYY).
Supervised Fieldwork Hours
Your actual supervised fieldwork hours: independent (A) and supervised (B).
Concentrated Supervised Fieldwork Hours
Your actual concentrated hours, without the multiplier.
Percent of Hours Supervised
Supervised hours divided by total hours, for each fieldwork type. Filled in for you.
Total Months Obtained (2022 form)
How many months of each fieldwork type you completed.
Qualification (2022 form)
The responsible supervisor's qualification: BCBA/BCBA-D, Verified Instructor, or ABPP/ABA.
Fieldwork Type (2022 form)
Supervised, Concentrated Supervised, or Mixed when you completed both. Ticked for you from the hours you enter.

Questions about the final form

When do I fill in the final form?

At the end of each supervision relationship. The individual supervisor form is one per supervisor, and the multiple supervisors at one organization form is one per organization.

Do I use the concentrated fieldwork multiplier on the final form?

No. The form says to report actual hours completed and not to use the multiplier for concentrated supervised fieldwork.

Where do the numbers come from?

From your signed monthly forms. The final form asks you to use your Monthly Fieldwork Verification Forms to report the hours you completed under each fieldwork type.

Who signs the final form?

Your supervisor. On the multiple supervisors at one organization form, the responsible supervisor signs and every supervisor who provided supervision there is listed. This page leaves the signature line blank for them.
