Questions about the monthly form

What goes in Independent Hours and Supervised Hours?

Independent hours are fieldwork hours when your supervisor was not present. Supervised hours are the hours when your supervisor was present, including individual or group meetings with them. The form adds them together for the total.

How is the percentage of hours supervised worked out?

Supervised hours divided by total fieldwork hours, as the form says (supervised/total). The generator rounds down to two decimal places, so it never shows more than you have.

Do I need one form per supervisor?

The individual supervisor form says: complete one form per supervisor. The multiple supervisors at one organization form says: complete one form per organization, signed by the responsible supervisor.

Can I sign the monthly form on this page?

Is the monthly form submitted to the BACB?
